Online Forms System

The Online Forms System enables students and staff to submit requests and information for various academic and administrative processes. The system also implements related workflows and approvals.

The Online Forms System integrates with University enterprise systems such as Callista allowing online forms to be pre-populated, reducing completion time and improving accuracy, resulting approvals are processed automatically into enterprise systems.


Staff must request access to a work queue for a particular group.

You can access the Online Forms System through the Staff Portal.


All issues should be directed to the IT Service Desk.
This includes issues such as:

  • the application is not responding; or
  • you are unable to access the application; or
  • or the application is presenting error messages.
